Keep all your doctors in the loop
One of the most important parts of managing your health care is making sure that every doctor you see has a full picture of your health. This includes your medical history, treatments, and medications. Keeping your health care team informed ensures that everyone is working together for your benefit. For example, medications prescribed by one provider could interact with those prescribed by another. Your doctors can prevent such risks only if they know exactly what you’re taking.
Keep your own records
While your doctors may share some information through electronic health records, it’s smart to keep a record of your own medical history. That includes any medications you’re taking, health conditions, surgeries, vaccination history, and other key details. Having this information handy allows you to quickly inform any provider you see, whether it’s your PCP, a specialist, or even a pharmacist.
Prepare for every appointment
Write down any new symptoms, changes in your health, or concerns you’ve experienced since your last visit. This is also a good time to review your medications so each provider knows exactly what you’re taking. Questions to consider asking at your appointments include:
